How do we get organisations, and individuals, to admit setbacks and failures to promote learning? 
How do we distinguish between setbacks that may lie on the road to eventual success, from clear indications that the enterprise is doomed? 
At first sight, there are no easy answers and there may not be any difficult ones, either!]]></description>
